Giardia lamblia
Giardia lamblia is a flagellate. These parasites move with the help of a tail and the flagella. Giardia lamblia attaches itself to the wall of the small intestine by a suction cup, causing damage to the cells of the intestinal wall.

Giardia lamblia infects 200 million people worldwide, causing diarrhea and abdominal pain. Annually, half a million new cases are reported. In the Netherlands, 5-6% of individuals with chronic intestinal complaints are infected with Giardia lamblia. The percentage is higher among children. Contamination with these parasites often occurs at (nursery) schools. The elderly can become infected in nursing homes and through contact with grandchildren.

Giardia lamblia damages the small intestine causing a decrease in the absorption of nutrients. This can result, especially with children, in weight loss or growth retardation. Infected children often have a pale face, dark circles under the eyes, they are listless, have smelly stool or do not sleep well. Minerals such as iron may be lost, causing anemia and fatigue. People with anemia and abdominal complaints are advised to take a Giardia lamblia test.

In combination with the bowel symptoms a rash, itching, fatigue, insomnia, nausea, headache, decreased concentration, flu-like feeling and sometimes fever may occur.
In severe cases malnutrition and dehydration can develop, especially with the elderly and children.

Irritable Bowel Syndrome
Patients with Irritable Bowel Syndrome symptoms have a 5-6% chance to be infected with Giardia lamblia. Based on the pattern of symptoms, one cannot discriminate between infection with Giardia or IBS symptoms.

Giardia lamblia DNA testing is available. The laboratory uses the qPCR method for detection of parasitic DNA. Since the parasite is not excreted every day, it is of importance to take a sample on a day of increased complaints.
In case of persistent abdominal pain, testing for Dientamoeba fragilis and/or Blastocystis hominis is recommended.

The results should always be discussed with a doctor. It is advised that family members are also examined.

Testing after the treatment is of importance, since medication does not always successfully eliminate parasites. Metronidazol is effective in 50% of the cases; it is important to know that resistance to metronidazol is increasing. 

Cryptosporidium spp.
Cryptosporidium can damage the small intestine and cause diarrhea. is responsible for 2-3% of the causes of acute diarrhea. Most of the time the symptoms of a Cryptosporidium infection will spontaneously disappear. Only individuals with a weak immune system, especially HIV positive patients, may suffer from diarrhea for a longer period of time. For people with chronic bowl problems there is a 1% chance to test positive for Cryptosporidium spp.
